| Physical
Examinations Physical Examinations are required of every student before entering 7th grade. It should be dated less than one year prior to entering 7th grade and submitted to the health office prior to the first day of school in the fall. During the summer, they can be mailed or dropped off in the schools main office. They must be completed on the blue HAR-3 form issued by the state of Connecticut. The form is available from the school nurse of main office. The doctor must complete all items which have an asterisk. The parent or guardian must complete the Health Assessment Record on the opposite side. |

| Immunizations All students must show proof of immunizations required by the state of Connecticut before they are allowed to enter school. 7th graders are required to have their second measles or MMR immunization and at least the first of the hepatitis B series. They must also show immunity to chicken pox. 8th graders are required to have completed the hepatitis B series of three immunizations prior to entry into 8th grade. Students entering from out-of-state schools must meet state of Connecticut physical exam and immunization requirements. They may be asked to submit a physical less than one year old. Students claiming religious or medical exemptions must present an immunization exemption form to the school prior to enrollment. |

| Re-admission
to school following Hospitalization/Emergency Room care. To ensure safety and appropriate care, any student returning to school after Hospital/Emergency room/Physicians care, must present a note from the health care provider. The note must include: 1.) The date that they may return to school. 2.) Any restrictions, limitations, or assistive devices (crutches, wheelchair, sling, cast, etc.) required by the student. 3.) The length of time they will be required. Students will not be allowed to attend school without this documentation. Students requiring assistive devices should be brought to the health office by a parent or guardian before attending class on their first day back. If the student is found in school without the proper documentation, the parent will be called to provide it or to pick the student up. |

| Emergency

Cards must be completed by the parent or guardian and returned to school within one week of the start of school. Please complete BOTH sides. The cards provide us with: 1.) Emergency contacts 2.) Permission to administer Tylenol or Motrin 3.) Permission to seek medical care in the event of accident or emergency. This is very important, especially on field trips. 4.) Parents are responsible for keeping this information up to date. |

| Illness In an effort to curb the spread of illness, please keep your child at home if he or she is ill. A student running a temperature over 100 or vomiting will be sent home from school and should not return to school for 24 hours. If a student is absent for any reason, their parent or guardian should notify the school by phone. The student needs to present a note a note from the parent or guardian on the day they return to school. If absent more than five consecutive days, we may request a note from the physician. |

| Gym Excuses If your child is ill or injured and needs to be excused from gym, a note from the childs health care provider is required. The nurse will excuse them for one day if she feels it is medically necessary. School policy advises that a child who is able to be in school should be able to participate in gym. |

| Medication Whenever possible, prescribed medications should be administered at home, before or after school. If a medication must be administered during school hours, we must adhere to state statutes. You will need to provide the following: 1.) An Authorization for the Administration of Medication form completed by the physician. Forms are available from the nurse or main office. Please note that the form must be signed by both the physician and the parent of guardian. 2.) Medications must be delivered to the school by the parent or guardian. They must be in the original pharmacy container. No more than a 45 day supply can be kept at school. 3.) The only medications students may carry are rescue inhalers and epi-pens for severe allergies. The authorization form, signed by the parent and the physician must still be submitted to the school nurse. *The self administration section must also be signed off by the physician and parent. 4.) Medication must be picked up by a parent at the end of the school year. Any medication left beyond one week of the close of school will be destroyed. |

| Health
Requirements for Sports Participation. Two items are required before a student may try out for a sports team. Both must be on file in the nurses office. 1.) A physical exam form completed by the health care provider. It must be less than two years old. (Required every two years.) (Download here). 2.) A health history form completed and signed by the parent or guardian. (Must be submitted yearly.) (Download here).. Forms may be obtained from the nurse, coach, or main office. Additional permission slips or emergency cards may be required by the coach. (Cheerleading is considered a sport.) |
